Assignment of Commissions

Are you wanting to still service your members but direct your payments to another agent/agency?
Request to assign commissions to another agent/agency by submitting a properly executed Assignment of Commissions form. More details about Assignment of Commissions can be found on the Assignment of Commissions FAQ document and in the Agent Guide.

Release

If a full release has been provided by your current upline please have your new upline include the release in their contracting submission. All releases must come from the top of the hierarchy so please work with your upline to be sure you have the required level of release prior to submission.

NOTE: Agent On-Boarding does not process change requests during the Blackout Period: September 1 – December 31.

Request to Successor the business to another agent/agency. More details can be found on the Successor Agent FAQ document and in the Agent Guide. If you have additional questions, you may contact the Successor hotline at 888-240-9165.
Completed Success Agent Interest forms should be emailed to sh_commissions_administration@uhc.com
